Ability for Facilitator to edit and submit Course/Facility Evaluation and Notes by Class #46

This lets a facilitator comment on what did/did not work re. the Course,
the particular Class, or the Facility (classroom) used for the course.

I think input needs to be collected in some standard form so that there is
some chance of running reports against the responses.  

Don may be able to suggest format.
