Section labels should be removed and added to the beginning of titles #184

Please move all the content within labels in sections to the beginning of the title.

For example:

<sec id="s1">
<label>1.</label>
<title>The role of the international journal of microsimulation</title>
<sec id="s1-1">
<label>1.1</label>
<title>The roots of the journal</title>

should be changed to 

<sec id="s1">
<title>1. The role of the international journal of microsimulation</title>
<sec id="s1-1">
<title>1.1. The roots of the journal</title>
