add connection dialog, update readme #53

add dialog to allow specifying the ip address on which to connect to a HoloLens and updated the readme/images to reflect ui changes

also:

fix incorrect comments
clear the status bar contents on a connection attempt
pass ConnectOptions object instead of individual values when connecting for future proofing the api signature
