Removed author information from every file
Fixed a few files that did not have copyright notice
Removed all TODOs

The folder structure looks like this:

cx sys xlat

I chose to have a cx folder in case we decide to also publish code that is under ndis (NdisRtl, PacketClient).

Can you please also add an rtl directory with KArray.h, KDebug.h, KIntrusiveList.h, KMacros.h, KNew.h, KRegKey.*, KSpinLock.h, KWaitEvent.h, and nblutil.h
