Make everything work with Azure Pipelines #2

This work:

adds build scripts and associated documentation
makes fixes to SimpleProviderManaged to run better in the test environment
introduces fixes to get the tests to run in the Azure Pipelines environment

This PR is the same as #1. I abandoned that one and started this one to test integration with Azure.

@cgallred apologies for the delay, I still need to finish reviewing the changes in this PR. I plan on getting through them this morning.

@cgallred the changes look good, as mentioned in the comments I'm not sure about any licensing concerns from the 3rd party logging library in the same provider.
