Create Issue + PR Templates #9

Currently issues and PRs that get opened don't have a standard format / consistency. To help with community participation we should create templates for both of these.

For issues template think I like how vscode has theirs setup

For PR template we can use typescript - theirs is commented out because they do something else now I guess but we can make use of it still
