After some digging I discovered #39 and that in order to create a new session from ad hoc work items (rather than a sprint or query) I have to select those work items first. It would be nice if the Estimates extension told you this when you select the "Work Items" mode and there not selected work items in the queue. Something like the following would be really helpful.
You must add work items to the queue in order to create this session.
