Replace pencil with globe icon and plus with hash.

Should we use a web browser icon like the globe you have for open? That arrow looks like "Redo" :)

Yeah, let me drop into the UI and see how it looks.

A few more. I do like the globe better than the redo arrow we've adopted in VS. I'll commit that here for now.
