Update extension to include work item form group sample which consumes work item form service

Readme.md needs to be updated with information about this sample. If it doesn't "just work", i.e. the work item type needs to be customized in order to see the UI, then those instructions should go into the readme.

Updated Readme. Group extension just works, it doesn't need any configurations.
