Fix for AKV signing and update docs #155

The PR fixes the submission of proposals signed with an AKV member key. We were missing the voting on proposals. Small edits to docs.

How can KMS members vote to a proposal that is created by another member? Is it future work or already covered by the current code?

This change has been tested on a clean mCCF instance. Follow the following guides to do a proper deployment and tests: Create AKV certificate if you need to create and register an AKV certificate on a mCCF network. Deploy kms to deploy KMS and to do basic testing
