Connections from ADS are not identifiable in pg_stat_activity #438

Describe the bug Connections made from ADS are not identifiable in pg_stat_activity (i.e., application_name is blank). ADS should add the "Application Name" parameter to its connection strings and supply some identifier for connections made from ADS, similar to most other PostgresSQL clients.

Using version v0.4.2 of the extension.
