More descriptive application_name in pg_stat_activity table #468

In pg_stat_activity provide more descriptive application name to describe use of connection. See how DBeaver client formats application name in #438. https://github.com/microsoft/pgtoolsservice/pull/477 propagated the application name which is currently a constant: 'azdata'
