Adds Deployer extension.

Adds Deployer extension for Azure Data Studio.

• Primary use is to execute SQL files against a connection in sequence in order to "deploy" them to a database.
• Includes other convenience actions:
  • Select from Table via context menu
  • Compare the stored procedure from a SQL file against a server version
  • View a server stored procedure definition
