User account ‘x@y.com' not found in MSAL cache, please add linked account or refresh account credentials.

This problem has been persistent for a few years, but has gotten worse. None of the solutions on related tickets have helped. https://github.com/microsoft/azuredatastudio/issues/24104# https://github.com/microsoft/azuredatastudio/issues/24894

Not a duplicate, because the other threads ask for a new issue to be opened if this recurs.

Steps to Reproduce:

  1. Connect to an Azure database.
  2. Continue working.
  3. Connect again within a few minutes to another Azure database. See error.

Or

  1. Reveal details (click wedge) of Azure SQL Database in the explorer.
  2. See error.

Yes, I have been assiduously clearing the MSAL cache for years, and the problem continues. Would really like to be able to use this tool!
