This PR adds the following:

An IScaleProivder implementation for use in the Azure Functions Elastic Premium plan
A new dt.GetScaleRecommendation() SQL function for more accurate auto-scale recommendations
Configuration settings for max concurrent orchestration and activity executions
New documentation topic for scaling
Various misc. changes (see CHANGELOG.md)

TODO before merging:

[x] Scale testing in the Azure Functions Elastic Premium plan.
