Customer report: ID is not set when posting a FHIR resource to FHIR server #12

Customer reported that we do not properly set the ID when converting to FHIR. We should investigate.

I think we should store all Identifiers of a resource under extension in HealthVault with some standards. It should include the HealthVault identifier too
