A spec on how semantic and index pairs are assigned to entry point function parameters, how signature points are identified, and how diagnostics are emitted.

This PR was based on a branch made directly on the microsoft hlsl specs repository, and is no longer accepting commits. I'll close this PR, and move its progress over to: https://github.com/microsoft/hlsl-specs/pull/216
