`Explain using Copilot` Quick Fix dissappears after selecting it once in integrated terminal

Type: Bug

1. Run a command that fails, like cargo test.
2. Select the Explain using Copilot Quick Fix to the left of the command prompt.
3. Try selecting it again.

For me, the Explain using Copilot Quick Fix dissappears after selecting it once in the integrated terminal.

This happens even if I comment out my entire ~/.bashrc file, so I don't think any custom configurations there are causing this issue.
