Issue Type: Bug

I do not know when the issue begins. However it seems to happen after you open, and change a file you can save it or not.

At that point you can no longer rename the file. Once that happens, you cannot save any file whether it has been open or not.

It also might be a "time that VS Code has been running"..

If I click and ctrl-drop to copy, I get a file with a .1 in the name... only sometimes I cannot change that name. I am not sure I edited the file previously - either way it is saved and closed.

The problem ends when I restart VSCode

I can not reproduce this. Can you please provide exact reproducble steps I can follow to see this on my machine. Apart from reproducable steps a gif showing the issue would help
