Details on using "nuget.exe add" to install packages in fallback folders for Toolbox #28

Added some additional details about using "nuget.exe add" to install a nupkg into a fallback folder, including advice on how to do that from an SDK installer.

Also removed Preview 3 known issues that were fixed in Preview 4.
