Moodle 32 dev - New features: apply "immediate feedaback" mode and provide ability to students to see their responses #56

Hello, This pull request actually replace previous #49 (February 2017) (https://github.com/middlebury/moodle-mod_adaptivequiz/pull/49) and cover proposed enhancement #43 (January 2017) https://github.com/middlebury/moodle-mod_adaptivequiz/issues/43 same new features + bugfix On my Moodle instance it works even up to 3.3.4 (3.3.5 has not yet been tested)

I want to propose given in title two new features:

"immediate feedback" mode
ability to students to see their responses

Also, I have tried to apply general quiz module processing order. Several minor bugs was fixed too.
Code is compatible up to Moodle 3.2.3
