Create a middleman template project for AsciiDoc that can be used to initialize a new Middleman site with AsciiDoc enabled.

I've created a template: https://github.com/opendevise/middleman-asciidoc-foundation-slim-template

Technically the example repository is also a template, so I'm going to leave this issue open until I polish it up to be used as a template: https://github.com/opendevise/middleman-asciidoc-example

I also need to create a branch in the template repository for middleman-4 that demonstrates how to use the middleman-asciidoc gem once Middleman 4 is out.

Middleman 4 is out. I think we should focus on Middleman 4 exclusively since we actually have control over that extension.

I haven't decided if we should have a separate template that integrates the blog, or whether we should just throw it all together.

The template still needs a lot of work. We can port ideas from the following two older efforts:

The hard part is always deciding what to include and what not to include.

@tdreyno Can we graduate this template to the middleman organization?

I'm still looking for ideas of what to include in the template. I have the basic framing up, but there is little in the way of styling. Perhaps we need some more styling, but I'm hesitant to add any sort of CSS framework as that should really be up to the site designer.
