Update multielements.txt to match the new font added in 397ac722. #2

With the updated Multielements font from issue #1 the codepoints for the glyphs were changed. multielements.txt needs to be updated to match the new code points.

I added a couple of notes to the file and changed those affected lines a bit. Thanks.
