Add mssql sample e2e test

This PR adds the mssql-app sample e2e test and makes some necessary modifications to migration_run role:

• When staging is set to true, migration_run will process it and perform a stage migration, then resume with the final migration
• The final migration will always be stage: false, since any staging has to be processed prior final migration
