Change name to 'couchappjs' #75

Since this project's interface differs so much from the Python implementation, their sharing the command-line name couchapp breaks projects built for one or the other. This PR changes that name to couchappjs so npm link exposes the command so it doesn't overlap / overwrite the Python implementation.

+1 This would save so much time and angst. There's so many variations on "couchapps" now, that they can't all claim the name on the command line.

Would be much appreciated, and would help the community grow rather than discussing how each of the tools isn't quite the same.
