Add estimated age to date workflow #8

Ask year of birth on student demographic page

Do we really want to ask kids the year that they were born? I expect most will not know. Shall we ask age instead, and then ask if the age is estimated?

Suggested flow:

Do you know your birthdate? y-> enter birthdate n-> do you know your age y -> enter age n -> enter estimated age

birthdate birthdate_known age_estimated

Then, if we only have an age we still calculate the birthdate (if the child says they are 6, assume they are 6.5 and populate the birthdate field appropriately). The known and estimated fields would probably never be used, but let us know when the birthdates are not precise.

Birthdate is usually entered from a school managed register. For now I will leave it as is, year/month with unknown fields.
