添加封面上传 #14

需要给课程增加封面功能，封面保存在七牛，实现该功能需要使用 https://github.com/mindpin/file-part-upload 的 2.0.0-beta3 版本 的 qiniu 模式来实现，使用说明见 https://github.com/mindpin/file-part-upload/blob/2.0.0-beta3/README_QINIU.md （估计只能DD做）
