This PR adds proper documentation for the full API.

The major code changes are mostly due to issue #45, with some other small adjustments/improvements and housekeeping tasks picked up along the way.

See UPGRADING.md for breaking changes required for the bug-fixes.

Regarding something that wasn't changed in the README: One of the first paragraphs states that a goal is to make it easy to switch between MySQL and PostgreSQL.

You mean this one?

An important non-goal of this project is the ability to switch from one database technology to another

As I recall, the ability to switch was always a stated non-goal for this project :-)
