Provide Better Error message when a Migration fails #51

Can we print things like:
SQL that was being executed?
The Migration number?
Any other random information that would let someone figure out what's going on.

Improve the logging of Error messages a bit. Print the stack trace regardless 
of the 
'trace' setting since these are exceptional events and not really debugging 
events.
