What does this pull request do?

Adds the new education F2F screen to the assign case page.

New education F2F Screen - With comments

This page is only displayed when:

The case category is "Education"
There are no valid providers
There are no suggested providers
There are no providers assigned to the case

Functionality changes

The submit button is changed to "Assign F2F help", this submits the Alternative Help form rather than the Assign Provider form.
The button functions similarly to the "Assign F2F help" button on the alternative help page but assigns the case the code "EDFF".

Any other changes that would benefit highlighting?
