Update formats #43

Add trigger_service_params & service_refused.
Add string_of_type for message types.

Split out from #39.

hmm, why splitting up #39? I had the impression it is nearly ready to be merged!?

I don't know; I haven't had time to look at it. I think it's the third PR with these commits in it, so I was planning to review and merge them separately to make the rest easier to review. If you think #39 is ready, feel free to merge that instead.
