Misc doc changes #119

Besides other documentation changes, this commit ensures the generated HTML doc for HexDocs.pm will become the source of truth for this Elixir library and leverage on latest features of ExDoc.

Thank you for submitting this- it looks like there are some nice changes here and I appreciate you taking the time to put this together.

There are several things I would format or word differently, so if you could grant me access to modify your branch (as described here) I will make a few additional changes before merging.
