Ok, I made a few changes to the login code 😄

I fixed some issues with non-2fa-enabled accounts, so you can now log in with either type of account.
I implemented refresh token support.
It now persists the access/refresh tokens to disk, so you shouldn't have to re-enter your 2fa auth code each time you restart Homebridge.
