According to Ned, this string is what happens when an empty string is passed to the translation engine and it can't find the expected translation. So perhaps this is caused by an error in the template. We don't see this on edge.edx.org but that installation may have more translations installed than our residential installation.
After passing results to an embedded LTI application, the LTI Consumer block displays an unexpected div similar to the following:
