Purpose of code changes on this branch:

Make it more clear what is going on when an IOException is thrown by creating 
our own exception type with better messages

When reviewing my code changes, please focus on:

Are there other spots where we should be throwing this kind of exception?

After the review, I'll merge this branch into:
