Fatal: ASSERT failure in QList<T>::at: "index out of range", file /usr/share/qt4/include/QtCore/qlist.h, line 387
occurs when a row is added to the bindings table that has empty string values or NULL for miditype, midino, and midichan.

It works okay if you put something in the strings.
