add on_cleanup callback for major errors like lost connections ensure that failed requests get ->croak calls -- including when a connection is broken before or during clarify pubsub logic

I meant to ping other devs, not like moving the code to someone's repo.

@dgl and @leedo have direct write bits to this repository and moving stuff to some other repo doesn't change a thing.
