Extension compatibility issue with Menu Editor - duplicate menu items

What steps will reproduce the problem?
1. Install Menu Editor Extension v 1.2.6
2. Select text and right-click selection - "Search with Google" default
activity is shown
3. Select text again and right-click selection - "Search with Google"
default activity is shown TWICE

Repeating step 3 (or 2) will increment the menu with yet another default
action EACH TIME. (see attached screenshot)

This is also true for all default accelerators (I tried Google Translate)

Extension: IE8 Accelerators v 0.7.4
Conflict with: Menu Editor v 1.2.6
FF 3.0.7
OS: Windows Vista HP

Unfortunately there is nothing I can do to fix this problem. I believe it is a 
bug in
Menu Editor. I wrote them email and let them know about the problem.

They are making bad assumptions about how the context menu works.
