If you change the primary shortcut of one of the default actions
eg. Refresh to a shortcut with a modifier eg Ctrl+R and don't assign
an alternate shortcut the button will be missing - OK that's fine as
you could customize some other button, but if you don't do that the
missing button leads to an NPE when you press an modifier to invoke
some other shortcut eg. Shift+F6/Rename and therefore to succesfully
disabled shortcuts :-(
