Currently, new cells are always appended to the end of the document when created, and may only be created using a hotkey (actually two hotkeys, Cmd+Enter to create a new code cell and Shift+Enter to create a new text cell). Instead, make it possible to append a new cell immediately after any existing cell by adding an "append new cell" button to each cell. (Maybe this needs to be two buttons, one to append a new code cell and one to append a new text cell.)
