Summary of Changes

Add Node Foreman to manage env variables
Add utilities folder
Refactor Angular structure
Add routes to /api/us/*
Add mongodb schemas
Add basic drop down menu and table
Add dynamic stacked area chart using Angular-nvd3
Add basic bootstrap

Looks good to me, there's a whole lot to look over though so if someone else could take a peek and sign off on it that might not be a bad idea in case I missed anything. Otherwise good to merge!
