replace ml5 glossary google form link to a formal one #101

currently, we use a placeholder created by @QuinnHe and ml5 mail list won't receive any notification. We may need to create a new one with ml5 account and replace the link. cc @MOQN @alanvww

@shiffman, when we discussed this, you introduced us to take a look at an alternative method. Could you please remind us of it once again? Also is there an official ml5 account for creating Google Forms?

Hmmm, I seem to have forgotten whatever suggestion I might have had! I don't believe we ever received any submissions via the form so I don't think it's a big priority to keep it. If we want to have a form, there is no official ml5 account so you could just make it with your NYU account and share permissions with the team.

Also, I noticed the page references an "Edit Document button on the top right" which should also probably be taken out.

We can refer people to making a PR for additions to the glossary instead of the form or an "edit" button?
