Add variable for origin of the process in birth-death process [sf#39] #51

Reported by hoehna on 2013-11-11 10:41 UTC The birth-death process should have a variable as a parameter specifying the time of the process. The users should be able to apply a prior distribution on this one and not implicitly assume a uniform prior.

However, check if there should be an option to suppress the variable and let the process start at the mrca.
