You can action surge twice between rests starting at 5th level, three times between rests starting at 13th level, and four times between rests starting at 17th level, but only if you did not use it last turn.
I think it would increase clarity if this is moved to a separate sentence such "...at 17th level. You can only use action surge if you did not use it last turn."
The way it is now it looks a little like the 17th level is when this clause comes into effect. Another option would be to move it to the 5th level section.
