These folks are rather difficult to get a hold of. My only contact with them so far was the day I discovered the issue, and that day took me about five phone calls before I got through to a human.

Their details are:

After going through phone circles for ages, I managed to get to the fraud department, whom could help. They did the following:

I'm still waiting for the investigation forms to arrive, which means they're not coming. I need to follow up.

Hours for the 6432 number appear to be: 8am-8pm EST, except Fridays

Have to call back on Monday and ask for the Investigations department. Apparently they haven't begun the investigation yet even though they were supposed to start it and send me a letter back on May 12th.

The hours for the investigation department are: 8-4:30 est, Monday-Friday.

On May 12th they opened a fraud investigation. That will take 60 days by statutory requirement, meaning the latest it will finish is the July 12th. I need to send them the police report and FTC Affidavit during this investigation. Unfortunately, nobody told me this until now.

Also, the bank has changed names. The address for investigations with the new bank name is:

Synchrony Bank Attn: OH3-1030 PO Box 8726 Dayton, OH 45401

The phone number to reach the fraud department directly is 877-969-1126.

Once the fraud is determined to be true, this should be removed from my credit file automatically and I will be sent a letter stating that indeed it was a fraud (resolving issues #6, #7 and #8).

If I wish to resolve issue #9, I must take that letter and send it to the debt collector to get them off my back.

Well, good news. The bank has concluded that this was indeed a fraud. They state that they have advised the three credit agencies to remove the account from my credit reports (which will take 4-6 weeks, apparently), and that "all charges have been removed and I won't be held responsible for the debt."

The debt collector has made sinister intonations that once they own the debt the bank is not relevant, so who knows if this will automatically resolve issue #9. So...off to deal with the debt collector I go.
