Add v2 output #4

It might make sense to add v2 XML output as well to mmark2, as the v3 tool chain for the IETF is still in development. As a new render is relatively easy to add this should be doable. And a lot of stuff could be copied from mmark1 (https://github.com/miekg/mmark)
