Deprecate xml2 parser and renderer #72

xml2 output for the IETF has already(?) been deprecated and the RFC editor has moved to some form of 7991 XML as the canonical form.

I'll try to get some authoritative source to say so and then delete this xml2 code in Mmark.

I have to double check, but I think you can now submit xmlv3 to the IETF, meaning we can drop all code related to the v2 output.
