What steps will reproduce the problem?
1. Using .xla from sforce_connect_20130503.zip
2. Errors on Auto_Open expecting to find a Sforce v3.0 object reference

What is the expected output? What do you see instead?
Can this possibly be updated to use office toolkit 4.0 references?

What version of the product are you using? On what operating system?
sforce_connect_20130503/ Office Toolkit 4.0 / Win7 x32
