Add issue templates #129

Design Details

Add github issue templates. Ideally:

Task/Story Template: Should contain a "Design Details" section and a "Done When" section
Bug Report Template:
- description
- steps to reproduce
- environment specs ( and/or instructions on how to gather them - probably would be a good section for the docs that could be linked to in an issue template).

Done When

Issue templates have been created.
