new form triggers 404 #257

ODK Aggregate 1.0 Beta 2 (plus PostgreSQL and Tomcat) test server running on

The 404 when clicking new form or upload data is a known (and I believe fixed) 
bug.  You have to add the tomcat ODK application name to the URL if you want a 
manual fix.
